Patient Information
Your first visit to the Center for Dermatology establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your provider.
Please bring the following information to your appointment (both new and returning patients):
- Medical Insurance Card
- Photo Identification
- List of medications related to your skin condition
- A parent or guardian if you are under age 18
If you are a new patient, please arrive 10-15 minutes early to complete the patient registration forms and medical history form.
If you are a returning patient, we will ask you to update your demographic and health information annually.
